Consent to the collection, processing and use of personal data

1. In compliance with the requirements of the Law of Ukraine “On the Protection of Personal Data” dated 01.06.2010 № 2297-VI, by signing this text, I give consent and permission to the Charitable Organization “Charitable Foundation “Wave 91 UA”, Taxpayer Identification Code: 44687338, to collect and process my personal data provided by me from the primary sources for the purpose of carrying out economic activities, in accordance with the current legislation of Ukraine and the Charter of the enterprise.

2. I agree that the process of processing personal data includes the commission of any actions and/or a set of actions, such as collection, registration, accumulation, storage, adaptation, modification, renewal, use, and dissemination (distribution, sale, transfer), depersonalization, destruction of personal data, including using information (automated) systems.

3. I confirm, agree and agree that the Charitable Organization “Charitable Foundation “Wave 91 UA” in carrying out its business activities, in accordance with the current legislation of Ukraine and the Charter of the enterprise, has the right to distribute personal data, place them on the company's website, provide access to the processing and transfer of information of my personal data to third parties without any additional written notice, without changing the purpose of their use and processing.

4. When changing my personal data, I undertake, as soon as possible, to provide the responsible person of the Charitable Organization “Charitable Foundation “Wave 91 UA” with the specified information and the originals of the relevant documents for making changes and/or additions to the personal database.

By signing this consent, I certify that I have received a notification about the inclusion of information about me in the database of personal data, about the owner, the composition, and content of the collected personal data, the purpose of collecting personal data and the persons to whom it is transferred, as well as about the rights defined in art. 8 of the Law of Ukraine “On the Protection of Personal Data”.
